The PT Rapidly Growing Role in Wilderness Medical Care
Increased participation in wildfire management and backcountry sports and recreational activities has substantially elevated the frequency and impact of medical problems in remote, challenging locations. Based on the successful experiences in the U.S. Military and National Park Service, strong arguments can be made that physical therapists (PTs) can be of great value during backcountry medical emergencies and as members of search and rescue teams. This 90-minute presentation will use lecture and discussion formats to integrate PT for the assessment and treatment of injuries and illnesses that are commonly encountered in the backcountry. Emphasis will be placed on the primary and secondary examination for medical emergencies and strategies to support patients and rescue personnel when there is a need for extended onsite emergency care time. Presenters will provide case study examples based on their experiences providing backcountry patient care in different capacities, including as deployed military PTs, volunteer Park Rangers, and as members of civilian search and rescue teams. Participants will be encouraged to develop innovative evaluation and treatment approaches within the context of wilderness medicine guidelines.

Dr. Hearn holds a DPT from the University of St. Augustine (2007), an MPH from UNC, Chapel Hill (2016), and a BS from the United States Military Academy, West Point (2002). He is an Orthopedic Certified Specialist and has practiced in various settings, with the vast majority of his experience being with outpatient orthopedics and as part of human performance teams. Dr. Hearn has proudly served the US Army in various positions since 2002, most recently as the deputy director of human performance at Fort Bragg, NC. His academic experience includes teaching classes on the evaluation of the complex patient, evidence-based practice, pathological gait, and health promotion. He is passionate about engaging with students and using data integration to improve performance, therapeutic approaches, and education.

After completing her Doctor of Medicine (MD), she completed her Emergency Medicine residency at Wake Forest Baptist Medical Center and then her fellowship in Administration, Operations, and Quality at the University of Colorado. While pursuing her fellowship, she also completed her MBA in Health Administration. Dr. Wendel continues to take a particular interest in health administration and quality and will be influential in her endeavors here at UVA Health and at the University.

Spinal Orthotic Indications and Application
Spinal bracing is a traditional treatment of spinal fractures, post-operative support, and back pain relief. The design of this course is to expand the physical therapists' knowledge base on spinal orthotics and its relevant applications. This course will discuss spinal orthotic clinical indications and associated diagnoses. This course is also designed to review trunk muscles response to spine injury and the therapeutic application of spinal orthotics as a conservative care evidence-based treatment. We will also review common clinical misconceptions about spinal orthotics that often limit usage in the clinical setting.

Introduction to Diagnostic Ultrasound and Applications in the Tactical Athlete
An informal hands-on course covering the basics of diagnostic ultrasound and applications in an acute tactical outpatient setting. This course will provide a fundamental overview of diagnostic ultrasound, applications, and basic examination procedures and allow participants the chance to get their hands on some devices to begin developing psychomotor skills. With the onset of virtual conferences, death by PowerPoint has become a pandemic. The Emergency Department as the Provider of Choice for Low Back Pain
The emergency department (ED) has been identified as a portal of entry for a substantial number of patients seeking care for acute low back pain (LBP). Current estimates indicate that between 4-5% of all ED visits are for LBP. Rarely, however is acute LBP an emergent condition. Information on the quality of care for the treatment of LBP in the ED is limited. Existing evidence shows a tendency toward frequent diagnostic testing and medication use, with two-thirds of patients receiving an opioid during their ED visit. Currently, we have little information regarding the trajectory of care for patients following a non-work related ED visit for LBP. This presentation highlights the care being delivered in the ED for patients with LBP and examined the association between guideline-based care (i.e., concordant) during an ED visit and subsequent health care utilization.

This session will describe some of the major differences between the way healthcare is provided in the United Kingdom and some of the challenges that this can cause. It will explore the role of physiotherapy in the emergency department, describing one service in particular. It will discuss some of the challenges that the British health and social care system can cause. The session will also discuss the role of advanced practice in the UK and what this consists of and what additional skills physiotherapists can obtain.

Physical Therapy in Wilderness Medicine:
An Emerging Field
The field of Wilderness Medicine focuses on providing emergency care and first aid in remote settings where help and resources are limited. This multidisciplinary field has developed from knowledge gained through Military and Tactical Medicine, Disaster Care, Search and Rescue, Sports Medicine, Exercise Science, and Survival Schools. As more and more people participate in outdoor activities, the number and types of emergencies continue to increase and provide a unique opportunity for physical therapists interested in back-country patient care.
This presentation will address 2 potential areas in which physical therapists will be of great value for wilderness-related medical emergencies: 1. as volunteer members of onsite medical teams where physical therapy skills in patient assessment, management, and transport of people can be of great value, and 2. as professional caregivers for patients who have sustained injury or illness in a back-country setting that will benefit from activity-targeted rehabilitation. Clinical examples will be used to illustrate these potential roles. These examples will discuss scene safety, primary and secondary assessment, unique concerns that arise when definitive care is delayed, and long-term complications of unique wilderness-related injuries such as envenomation, lightning strikes, cold injury, and infected wounds.

Dr. Beattie is interested in determining how findings from spine and brain magnetic resonance imaging relating to the clinical presentation of back pain. His team performs data collection at the McCausland Brain Imaging Center at Richland-Palmetto Hospital and was recently awarded an RO1 grant to assess differences in brain activation during exercise between people who do and do not have current symptoms of pain. His second area of interest relates to the prevention and treatment of lower limb bone stress injuries. His team is currently collaborating with the United States Army Research Institute of Environmental Medicine (USARIEM) in a prospective cohort study at Fort Jackson in Columbia, SC. This study uses a variety of physiologic and behavioral measures to identify individuals at elevated risk for bone stress injury.

Discussion over adaptation of our minor injuries unit from a nurse to advanced physiotherapy-led service during six months of the Covid-19 pandemic. This included the redesigning of rota’s, rescheduling of job plans, redeployment of staff, extended working hours, and extension of clinical skills and responsibilities. We will discuss the inclusion of orthopaedic surgical staff into the unit per the British Orthopaedic Association Covid-19 recommendations, broadening the MDT, and the opportunities presented, including audit data.

Return on investment for PT in the ED
This is a summary of the efforts that went into getting a PT in the ED at the University of Maryland Medical Center in Baltimore, MD. It reviews the process improvement study conducted to determine what impact PTs in the ED would have on ED length of stay, hospital admissions, and return on investment for the hospital. This effort was a collaboration between rehabilitation services, an ED physician group, and statistic analysts with the University of Maryland College Park School of Business.

Drs. McDonnell and Wendl practice in the Barnes Jewish Hospital, a Level One Trauma Center located in St. Louis, Mo. We provide services for a large underinsured population located in the inner city. The Barnes Jewish Hospital Emergency Department is a 70-bed facility. Debbie and Pamela will present exemplary cases that reflect the types of consultative services provided by physical therapy in the emergency department. During the presentation, we will highlight the primary findings that lead to our decision making treat/refer to physicians. Participants will see a variety of patient types ranging from true musculoskeletal to medical and neurological. The treatment used in each case will be presented. Differential Diagnosis of Patients Presenting to the Emergency Department with Dizziness
Breakdown of current literature involving differential diagnosis of patients with a primary complaint as dizziness. Poor reliability of patient's description of dizziness, with growing evidence to support the use of patient's history, as well as timing and triggers of dizziness. Review of appropriate bedside testing, as well as identification of red flags warranting further medical work-up / imaging. Introduction inappropriate selection of repositioning maneuvers and initial treatment for other vestibular pathologies.

Make Room for Wounds
Despite the growth of emergency physical therapist practice, barriers to EPTs providing care for patients with wounds continue in an environment where many wounds exist. This session will discuss common issues and workable solutions toward including this important part of the PT scope of practice. The session will also cover some of the basic assessment and skills needed for commonly encountered wounds in EM, a review of dressing choices, and follow-up care recommendations with case examples.

Physiotherapy models of care in the emergency department for patients with musculoskeletal disorders: A review of the literature and preliminary results from Canadian pilots projects in the province of Quebec.
Over the past decade, physiotherapists have emerged as key healthcare providers in emergency departments (ED). New models of care, including either conventional physiotherapy or advanced practice physiotherapy care, have been emerging worldwide. ED physiotherapists can provide effective care to patients presenting various disorders, but one of the most common types of disorders seen by physiotherapists are musculoskeletal disorders (MSKD). Thus, this session will focus mainly on ED physiotherapy care for patients presenting MSKD. In the first part of this session, an update on the current physiotherapy practice in EDs will be presented based on the most recent evidence. The second portion will address the diagnostic and management plans of physiotherapists in an ED setting for the care of patients presenting MSKD. To date no studies were available regarding the diagnostic and management concordance between ED physicians and physiotherapists, as so, results from a new study regarding concordance between ED physicians and ED physiotherapists will be presented. Finally, the next steps regarding the development, implementation, and promotion of ED physiotherapy care worldwide will be discussed.

Psychologically informed pain management in the Emergency Department
Managing people in acute pain and identifying factors contributing to pain is an essential skill of emergency PT clinicians. People presenting to ED with pain are distressed and may exhibit behaviors such as catastrophizing, fear avoidance, and freezing/immobilization. Additionally, they typically have strong preexisting beliefs and expectations regarding what may be the cause of their symptoms, what treatment or special tests they might need, and whether or not it is safe for them to move. Successful ED encounters may require addressing these issues, however giving education and reassurance while being sensitive to patients' beliefs and fears can be challenging. This session will examine the patient encounter through a BioPsychoSocial lens and identify techniques and approaches that may assist emergency PTs in managing patients' fears and beliefs, effectively examining acutely painful individuals, restoring mobility, and appropriately educating the person immobilized by fear and pain.

Evaluation of Patients with Post-Concussive Syndrome in the EDT

In 2012, the Orthopedic Section of the American Physical Therapy Association developed clinical practice guidelines to "describe the peer-reviewed literature and make recommendations related to (1) treatment matched to low back pain subgroup responder categories, (2) treatments that have evidence to prevent recurrence of low back pain, and (3) treatments that have evidence to influence the progression from acute to chronic low back pain and disability"(JOSPT 2012). This session will review the acute low back pain subgroups within the Clinical Practice Guidelines. It will review the literature for treatment strategies for each subgroup. Finally, the session will discuss complicating factors within the Emergency Department setting that may lessen the impact of some interventions.

PT students in the ED: harmful or helpful?
The session will include a presentation of differing practice models of clinical education in the ED from current practices, will discuss the education of students during their academic programs regarding the practice of PT in the ED, and will discuss the selection criteria used by some programs for student clinical rotations. The session will also include strategies for preparation for students in the ED, the orientation of students, and strategies for evaluation of progress during clinical rotations using examples from current practice. The session will discuss the challenges of having students in the ED environment from both the student and CI perspectives.

Physical Therapy Culture Meeting Emergency Department Culture
Discussion about our particular program having difficulty recruiting acute care and outpatient PTs interested in covering the ED and what is the best schedule for an ED PT. Traditionally acute PTs are Mon-Fri 8-hour shifts with the occasional weekend and holiday, and outpatient PT is Mon-Fri with no weekends or holidays, with both ending around 1630. The ED culture is typically a 3-day-a-week position (RN/Tech work 3-12 hour shifts, PA/NP/MD work 3-8 hour shifts), and the shifts are typically busier from afternoon to late evening. How does everyone find success with schedule balance and PT recruitment?

Rehab in the Emergency Department: Combatting a System that Fails the Frail
In this dynamic presentation, participants will:
1. learn the clinical definition of frailty, and appreciate the significance of frailty and associated sequelae in the older emergency department population;
2. understand how the ED often fails to meet the need of frail older patients;
3. recognize the unique position ED-based rehabilitation practitioners have to advocate for frail older clients and to reduce their risk of adverse outcomes following an ED visit.

PT in the ED, where do you see yourself in 5 years?
Physical therapists have been practicing in the Emergency Department setting for quite some time now, with some reports as early as the 1990s. Recent trends indicate a growing number of physical therapists practicing in this setting, and research in this area is becoming more prevalent. However, the American Physical Therapy Association (APTA) has been reluctant to recognize this practice setting as a special interest group, and little education is being completed across the country in DPT programs. The first-ever PT in the ED conference is a tangible result of the recent growth and interest in this innovative practice.
I believe the key stakeholders of PT in the ED are at a pivotal point in determining the future landscape of this practice setting. We can continue to advance this practice area by identifying clear leadership roles, creating a mission statement, and defining our scope of practice which will ultimately send a clear and unified message to the APTA and our profession regarding what PT in the ED represents. Furthermore, creating membership could promote advocacy, support research opportunities, help organize events, and advance networking to share ideas and knowledge, among many other potential benefits.
There is no question this proposal is easier said than done. Establishing a formal identity with leadership roles and creating a membership group could entail countless hours of behind-the-scenes work to achieve, but the benefits far outweigh the barriers. Ideas are wonderful, but action drives change. The first-ever PT in the ED conference is the perfect place to act on these aspirations.
